That's impressive. Did I noticed a shift during the dyno run? I noticed on mine that the tow/haul mode doesn't lock it out of top gear like on older vehicles. Not sure how dyno operators have to run these newer trucks with 8 speed transmission and keep them in the 1:1 gear.

Was your issue happening in any gear after 5lbs of boost? I wonder if cylinder pressure is higher in the higher gears due to engine load and since boost will be the same regardless of the gear.

I haven't been into an SCT tuner for awhile but I thought that they had a logging function. I might be thinking of a different tuner but it may be worth looking into. If you can capture fuel trims, ignition and a few other parameters it may point towards your problem.
the two things I see are a flat MAP signal and rich 02 readings happening in 4th gear. Fuel tables don't know what gear you are in and I highly doubt that vehicle speed would be an adjustment variable for the fuel tables. It's hard to tell if the flat MAP signal is the root cause or whatever...
