you dont need the physical sub itself, any sub will do, just match the size and ohms of your sub with an aftermarket and you will be just fine

Take your old bolt to a proper commercial hardware and have them match it up

start looking for parts,,

You absolutely do not need a new crank shaft.. Just bolt on the new balancer and you will be fine... Dealer tried to charge me the same back in 2005 when that happened to me, i had them sand down the end of the shaft to remove burs and put on a new stock pulley, .. They bitched and complained...
