First Pic is of the Resurface Flywheel!

Second Pic is a close up of the "groovyness" of the pressure plate (note the blurry clutch hub in the foreground!)

Third Pic is of the burned pressure plate

Fourth Pic is a profile shot of the "faulty" clutch hub

Fifth Pic is of the "bad side" of the clutch hub

Sixth Pic is of the "good side" of the clutch hub

This sounds like an improperly bleed out clutch hydraulic system. Also, check to make sure that the clutch line is not too close to the exhaust manifold. If so the fluid boils air gets into the line.
