How can you start a poll?

I wanted to start a poll on who have had their master or slave cylinder fail. How can I start a poll? Was also interested in how many replaced the cylinders when doing other tranny work even if they did not fail in anticipation they would fail sooner or later.

When you start a new thread there is a check box at the bottom of Additional Options, to add a poll. It is right down near the bottom of the page.
