A few other forums have compiled lists of these fails, so I figured somebody ought to bring the fun to ChallengerTalk.
Here we go. Please link in other examples as there are surely countless failures floating around the net.

1) This user pours NOS energy drink into his/her gas tank thinking its fuel treatment.

2) Engine oil was dirty, so this user flushed crank case with water.
