Someone drop it on the floor before installing it? Doesn't look like the piston hit it but you never know. Is compression good on that cylinder?
 
It had bosch platinum 4's in before i did a tune up. 6 out of the 10 were messed up. Now with new oem plugs it did the same . No knocking and compression is good in all 10 cylinders. 91k all stock. It only does that at wot.
 
Carbon buildup on the piston top? How many miles? You might have to shim them so they get more clearance.
 
Sir, I would say your friend is experiencing detonation. I have had this happen in the past, and this was the cause.

Do you have access to a bore scope? Would try & look inside plug hole for damage. The missing pieces of plug worry me.
 
There's no way I'd take 91,000 Mile motor to wide open. Not unless I was looking for a reason to tear inside it.

i was thinking last night and this crossed my mind. With this mileage you could have dirty injectors that are causing a lean condition leading to detonatation. I would take the truck to a dealership for a fuel injector flush or at a minimum run a few cans of the fuel additive cleaner to the tank and change the fuel filter.
 
The Rod bolts in these engines are not weak. No rumors need to get started.

not starting a rumor, the ones in that truck could be sub standard, manufacturering tolerances out of whack. just because 10,000 sets work right doesn't mean 1 set can't fail.

either something (rod bolt, rod, piston pin) is allowing the piston to go too far into the combustion chamber OR the spark plug is too long (previous set and new set) or someone tried silicone for head gasket or super thin one (which i don't see accounting for that movement)

you really thin detonation caused the strap to be bent one ALL the pistons without really eating up the rest of the spark plug?? could be i guess

Apparently you don't read much on this forum... There are plenty of guys who have well over 91K on their motors and run them hard without issues. Some even spray generous amounts of nitrous with over 100K on the clock without issues!!!
